Hi Marquez Coaches,
To ensure we stay in compliance with the new requirements from the LAUSD Athletics Office, we'll be utilizing the updated forms. We need to follow these specific guidelines for each transportation scenario:
  1. Parent Driving Students: If a parent is driving students, they must fill out the #1 Marquez PrivateAuto Parent Driving Student form.
  2. Student Driving Themselves: Both the parent and athlete must sign the #2 Student Driving Themselves form.
  3. Paid Coach Driving Students: The paid coach fills out the #3 Paid Coach Driving Student form, and the parent and athlete must fill out the #5 Student Form Coach Driving Transportation Waiver.
  4. Volunteer Coach Driving Students: The volunteer coach fills out the #4 Volunteer / Coach Driving Student Private Automobile Driver form, and the parent must fill out the #5 Student Form Coaches Driving Transportation Waiver.
